I've heard of the term "insert slate" and pal labels.  Anyway, I think this is what you're looking for:

I use these on my cases:

http://www.avery.com/avery/en_us/Products/Labels/Shipping-Labels/Weather-Proof-Mailing-Labels_05526.htm?N=0&Ns=&refchannel=c042fd03ab30a110VgnVCM1000002118140aRCRD

They are surprisingly durable, and relatively inexpensive.  I run the labels through my color laser printer.  You could easily create your own template and handwrite contents, but you could also skip the handwriting part and print the whole thing and stick it on the case.  At 30 cents a piece, it's pretty darned economical.

I called Gator and they were more than happy to sell me as many as I wanted.  I think they were about $4.00 or so dollars per 2 years ago.

+1, I just had McLoone make some custom ones for me that I am very happy with.  You can give them you logo as well, which looks a little sharper in my mind than a stencil.
