Ignoring the obvious problem with the conduit, how do you even open the access cover? The hole for that meter is an unauthorized modification, preventing the cover from opening normally. Should hinge upwards...

Can't tell from the pic, but that might not be an install violation. That might be a slip meter riser that has had the bushing/locknut vibrate loose or some knuckle head unscrewed it and can't fix it without breaking the meter seal.
Meter base was probably just fine until POCO decided to use fancy new electronic remote meters. They really can't make a customer upgrade for the POCOs convenience and those new meters are a pain!!
