Ok. I'll try to get a photo up before the weekend.

To tell you the truth, it's been about 5 years since I ordered it, I really can't remember.

At one time, I had made some short assemblies for a similar application using 12-conductor SO cable to create four 3-wire circuits.  The receptacle end would include either a dual or quad duplex box (see below) and the dimmer end would terminate in a 12" fan to plugs.

Hello Ryan;

Daniel Woodhead is one manufacturer but they're pretty spendy.
They've been supplying these since the early seventies.
Personally, I installed the first one I met in a theatre where it's been withstanding the rigors of one-nighters since 1973 and is still going strong.
Any serious, dedicated, electrical wholesaler ought to be able to price Daniel Woodhead products for you and, by now, there are likely more affordable options to consider.
Originally these were only available in bright yellow but I think I remember Woodhead now offering black as well.

Hubbell and Woodhead both offer the same basic style, typically a back-to-back duplex.  Available primarily in yellow, but also black now.  I don't like either though as they're kinda bulky and of course, rather costly, in comparison.

Those boxes look like they came from OA Windsor http://www.oawindsor.com/. I purchased boxes from them to make Booth Stringers. They are very high quality and if I remember correctly a better deal then the Woodhead stuff. Ken is a really nice guy and will even share some tips on how he makes indestructible cable assemblies.

That is correct, I used to have more images available, but apparently they're no longer on the computer.

I've been working on a few industry specific design's with OAW for a few years.
Time permitting, perhaps, they'll come to fruition someday...

Joe,
Sorry for the delay.  Here's the photo of the snake.  Upon further inspection it looks like the wire is 14 AWG.  You can see the 6th wire that does not have a plug...you can add a plug and have 6 circuits if you like.  Let me know if you think it'll work for ya.
