Rob - are those ADAT optical passthroughs? I've been looking for a way to get my ADAT connections into a Neutrik D sized hole, or any other size for that matter, but I've been unable to find anything.

Yes they are. The upper right is the BNC for the word clock. That way I can use only inputs if I want to use all 24 ADAT outputs for recording.
What I did was this...
I bought some "D" blanks from Triktags.com (they were actually drilled for 1/4" jacks) and also bought some TosLink Couplers. Here is an example -
http://www.hosatech.com/hosa/products/GOP-328.htmlI then drilled out the blank for a tight fit on the coupler and used epoxy to hold it. I used short fiber jumpers inside to connect to the preamps.
At the bottom I just took a normal cheap rack mount power strip, removed the cord (on the other side) and filled the hole with a bolt and washer. Then punched a couple of holes on the outside for the Powercon connectors.
What I like about this is that there isn't any fishing around inside the rack for setup.
What is a problem is getting all this to fit
If you are too generous with the XLR cable lengths it is tough to close it all up.
