Let's talk electrical safety - Brazilian singer electrocuted on stage

Saw this in a Redit post - https://www.aol.com/singer-ayres-sasaki-35-dead-043612749.html

Article doesn't give much detail and I don't know anything about electrical codes in Brazil, but I couldn't help think of the bad old days when we sometimes got a shock if our lips touched the mic while our hands were on our guitar. Of course those old tube amps had 2 prong power cords.

So any theories how this could happen in today's world? What non obvious things do we need to be looking for when running shows?

John Roberts {JR}:
Backline should be protected by GFCI/RCD breakers. Stuff happens.

That's terrible - and certainly should NOT happen in this day and age.

Everything is speculation at this point. I hope the investigation reveals the culprit.

Reminds you how dangerous electricity can be.  I've seen many cases where corners get cut and because it still works, nobody seems to care.

I used to just let it slide, but no more. If there's an electrical safety issue, the show will NOT go on unless it gets corrected.

Last year, I had a major muti-day event and the power panel was a dangerous mess.  We were aware of it ahead of time, but they neglected to get it addressed.  I arrived and said it needed to be fixed or the show wasn't happening.  They ended up having to call in an emergency electrician to come fix it and paid a pretty penny to get it done.

I don't feel bad about it because I'm still alive today.  Had they addressed it when I first pointed it out, it would have cost a fraction of what they ended up paying.

When I did seminars on live sound I used to tell students that the answer to every question starts with "it depends" unless it is a question about safety.
