EW G4: guitar volume adjustments causing noise

We just purchased 4 Sennheiser ew100 G4 systems for our band's guitars and bass. Everything works and sounds great, EXCEPT we get a loud noise similar to static interference whenever we turn the volume knob on any and all of our guitars and basses through any of the G4s. Once the volume knob stops moving it's fine regardless of position. If we bypass the G4s and plug straight in, the volume knobs are all clean when they move.

We tried various combinations of sensitivity settings on the packs and AF Out settings on the receiver, but nothing solves it. The signal is otherwise super clean and full. It's only when we move the volume knob on any instrument that we hear static.

Is there something i'm missing? Thanks so much!!

Get a voltmeter and test to see if there is a DC voltage across the pin and sleeve of the 1/4 " plug.
It sounds top me like the adapter cable is not wired correctly.

Thank you so much Keith! It was indeed that the wiring was wrongly done for mic input and not instrument input. (A tech had made us custom cables using mogami/neutrik and we were planning on using the original cables as backups, but I did not have access to the originals when testing. A swap of the tip and ring connections made the problem go away, as per the diagram you linked.)
