DI box output to a transformer splitter?  I haven't seen a device as you describe although it wouldn't surprise me if it exists in the broadcast world, where $500-$1000 items the size of a cigarette pack seem to be very common.

Maybe but lots of $$$. I need 4 iso outs, stereo application, -10 preferred. .  2 in, 2 iso outs per input.

stereo Di box & link the Left & right Input? (or use 2 seperate DI boxes to complete isolate them)

EWI  MST- 412-A?  Four channels, dual iso outs for each channel. Rack mount.

This would be my suggestion as well. Something like Radial ProAV2. Either modify the box to do it or a short jumper cable between whatever set of connectors you're not using.
