There is a "Studiomanager" setting to the right of the Rx/Tx port settings. That's the one that should be set to USB 1.

I tried switching from post to prefade while adjusting a few other settings to see if mine would drop the prefade signal. It worked every time. Have you solved this yet?

Finally, I got things working now:

1) Concerning Studiomanager:

I uninstalled the entire software, and reinstalled it.
Then I used the datawheel (as Mac suggested) and not the enter button to set the USB port. Thanks Mac!

2) Concerning pre-fader monitoring:

It's working now but I have no idea why it was not working before. I had performed several times factory resets and tried pre-fader, but would not get any sound. Post-fader was working well. I performed another factory reset - and voil
