Alternatively you could make a barrel that uses bipolar capacitors on pins 2 and 3, which is what I gather is done internally on a lot of devices. This old schematic (pdf) has been floating around for a while.

You could put a 680 ohm resistor from pin 1 to 2 and another from pin 1 to 3. this will drop the phantom to about four volts. 680 ohms should be fine for a headphone output to drive.

The lurking snake-in-grass apprehension about using transformerless phone/laptop 3.5mm to XLR adapters has been in regard to unwanted phantom power DC backfeeding & cooking  host device.

Aha! Likely cause of a cooked iPod touch on a low-end console with global phantom.  Thanks.

How may this help OP? Does this prevent phantom from hitting headphone amp?

I was looking for a dead-simple passive method. A dumb "check valve " for DC.

I always use the Whirlwind inline ISO barrel. Quick and effective. Never had a reason to do anything other than that.

"Use a DI" or "plug into the 1/4 TRS" are the simple options. 

This is a normal everyday situation easily connected with normal everyday parts

What's the hard-on for the Stage Bug or similar items?  Cord too short?  VP in Charge of Nothing Important not like the color of the box?  Give 'em Bluetooth and let them have fun with the issues that will present.
