AVIOM an16/o Problems

Hello, we have a brand new Aviom an16/o that we are using to convert our d800 Mixes Out digital signals to analog XLR then going into our Sennheiser In-ear transmitters. At first, everything was crystal clear and working great, but after a couple of hours, the transmitters started peaking and all audio was unintelligible. We've checked and double checked input signals, direct out signals, cat5 cables, electrical connections, possible sources of interference, EVERYTHING. It seems to me the problem may be the unit itself.

Any thoughts?
