If going the box-cam route don't cheap-out on the lenses. Better ones won't be as fish-eye and you'll be able to fill the screen with more document image before lines start to bend. Make sure the lens chosen also has the auto-iris with the pigtail connector.
I've also done the bingo-thing with basic re-purposed consumer camcorders (from the end of the digital cassette era) which can be found practically dime-a-dozen and usually always had a real composite output.
We use an HD security box cam that has an sdi output that we convert to HDMI. Poke around bh photo video to get an idea of what is available.
