My next step is to use different cables, flip the switches back and forth a few times and also run the speaker cable directly from the amp instead of the patch panel in my rack.

I haven't tried different speakers because my other speakers are powered but if the above doesn't help I will certainly borrow some to test.

Yeah the patch panel 9and the cables going to it-could be another source of an intermittent short.
