"Live Nation Canada, Live Nation Ontario, Optex Staging and Services have each received four charges in connection with the collapse for failing to ensure the stage’s structural integrity. Engineer Domenic Cugliari was charged with endangering a worker as a result of negligence."

CBC News - Stage design in fatal Radiohead concert collapse called for parts that didn't exist

Forensic analysis of the stage collapse attributed its primary cause to crushing of the pick-up trusses. The stage roof grid was not strong enough to bear the load of lights, screens and speakers, which weighed roughly 34,473 kilograms (or 76,000 pounds).

The stage design, which the inquest heard had been in circulation since the late eighties or early nineties...
