Does anybody know the part number for the 4 position phoenix connector that is about 1/2" by a 1/4" with a rectangularish key on the top?

If it's actually a Phoenix connector, it's likely from the Combicon line.  I'd contact the equipment manufacturer for the part number, though, as there are both 5mm pitch and 5.08mm pitch versions, and they look the same.

Be sure to get the ones with the tail for cable strain relief.
