Phantom Pop

A musician was plugging her DPA violin microphone into an XLR line. When she did this there was a loud pop even though the channel was muted on the A&H QU mixer. Phantom was active on the line but I have never had an issue plugging in DI boxes with phantom applied as long as the channel was muted. Yes, you could see the signal lights peak on the channel, but never a pop through the system. The venue was an old historical building with non grounded AC outlets.
Any ideas how the pop made it through the muted channel?

Don't turn on the phantom power until after you have plugged in the mic.

Could the "pop" have been through the monitors or another mix output. Depending where you pick off the mix send feed it could be before the mute.

I often use a whirlwind micmute pp for close miced instruments .  I get pops on one fiddler's dpa mic.

That's a solution for the next time.
I'm more interested in knowing why, in this case, plugging in a device using phantom power caused the pop to get past the channel mute. As I stated, I have plugged in DI boxes and condenser mics with the phantom power engaged and never had a pop get through the system as long as the channel was muted.

Possibilities?
1. Something with the DPA wiring?
2. Bad connection on one of the 3 connectors on the mic cable or snake channel?
3. Non grounded AC at the venue?

I was only using mains output. No monitors.

Always turn off phantom when doing connections.
