I can't tell for sure from Keith's photo, but is it possible that one of the transformers was installed backwards (rotated 180°)?  There appears to be a little dot on the PCB that corresponds to a mark on the transformer bobbin.  They line up in Keith's photo, but maybe not on Jonathon's.

I was considering that and included the pic so the OP could compare.
It's the only way I can see of a polarity reverse other than an outboard cable or console channel setting issue.

I picked up one of these several months ago and have used it with Win7 and Win10 laptops without noticing any issues.
Will have to try your "mix-to-mono" proofing method to see if I encounter anything when I return from my current trip abroad.
