What assumptions were made ?  E.g.vertical dispersion of the box itself.

The only assumption is a typical 12" or 15" speaker with a height of appx. 24".
As it says in the notes, height to the of bottom of the box and centerline of pattern.
You need to account for your speaker pattern.
It's just a diagram to see where the speaker points at different tilts and heights.

Additional scales at 2' and 4' would be useful. That would reflect audience head height for a 8' speaker on stand with standing and seated audience.

Print it out and use a straight edge.  It's easy to approximate the distance since he gave you a 2ft increment from 8ft to 10ft...

Too many lines just gets confusing.
The floor is a constant.
As noted, account for head height.
