Not sure if this violates code

Most likely does if it is the main service panel-they are to be marked and readily accessible.  Hiding it makes it hard to consider it readily accessible.

In a public space the panel must be clearly marked. The real problem may or may not exist because I can't see what's behind door #1. The panel must be fully enclosed with a steel or fire proof enclosure. If you open the door then open the panel OK. If not the panel door needs to be put back on. In either cases these should be simple fixes.

This is just wrong period. Not using a wire nut, then taping the wire nut is just lazy. Screwing the wire to a wood beam, a source of combustion, is just stupid. Draw current, generate heat, start fire.

A few early fuse panels didn't have a door; just an escutcheon (for lack of a better term) to conceal and protect the wiring to the fuses. Don't know if that's the case here.

I have come across several edison fuse panel lately that needed no penny-the fiber insulators are old enough they are disintegrating. I used to feel like edison panels were safe enough if fused properly.  Given their age and my observations, my opinion is changing fast.
