I had to order that just recently. I got it from Full Compass. I don't remember the price, but it was not expensive and I got it very fast.

I have never had any problem with or quibble with the audio quality of the preamp/DI that is supplied with the model 4000 piano pickup.  I will also state that this is pretty much unique in my experience with Barcus Berry electronics.

A Countyman or other active DI is not an option in two of my upcoming shows as the 1/4" output with local volume control is part of the artist's workflow.

If you've got time during setup you might try a Countryman or some other DI with a very high impedance (Radial PZDI, Baggs, Fishman, etc). The Barcus-Berry box isn't a very good match for their pickup, and because of that loses some bottom and has a quacky-sounding mid.
