AV event Q & A form?

I've just started doing business with a catering company in a hospitality setting (hotel banquet & ballrooms) who has no clue when it comes to selling my AV services (sound, projection, etc).

Does anyone have a question & answer type form that you've built that would help a potential client choose AV packages and services? Then the caterer could simply have their client fill  out the form & pass it along to me.

Simplify with packages instead of a la cart equipment. We thought about doing some sort of flowchart thing - "if this, then you need this" but it got way to convoluted. All documentation should be peppered with "For needs not listed, just give us a call."

But yeah, cluelessness is amazing. One of the biggest sound productions I ever did in my life started with the phone call, "We need to rent a microphone."
